There seems to be a number of issues observed (that have changed over time) that seems to have the result that notifications via element X are lost or delayed (by minutes) unless there are a large volume of push events.
Observed symptoms in EX push history:
Confirmed the same happened on EXA 25.07.1.
For SchildiNext 0.10.4-ex_25_7_1, push history shows up as "Success - null". With high volumes (e.g. during spam waves), sometimes a "You may have unread notifications" is shown. Toggling the Chamber of Secrets > Notification worker on and off doesn't change the issue.
Push works perfectly on a Synapse account however. Perhaps push rules may be misconfigured/missing for cwt.

I have an iOS device running an older version of elementX (version 1.9.3 (134)). This version uses Continuwuity, and notifications pop up but don't make sounds. I remember some changes to notifications since Continuwuity enabled simple swipe syncing. In later versions of elementX, the notification bubbles disappeared, and the sound still doesn't work.
Unable to reproduce anymore on the latest version of Element X iOS Testflight (as of 2025-08-15). I'm assuming they fixed something on their end given the torrent of notifications I received over the course of two days.
Cannot speak for EXA
Still nothing on latest EXA version
The solution to this issue is to buy an iPhone /j
